The OARSI non-surgical management guideline evaluated 60 interventions with GRADE methodology and built its recommendations around core treatments - structured exercise, weight management and self-management education - for every patient with knee osteoarthritis, regardless of what else is added.

In a 480-patient, four-arm, multicenter randomized phase 2/3 trial, none of three orthobiologic cell injections - autologous bone marrow aspirate concentrate, autologous adipose stromal vascular fraction, or allogeneic umbilical cord tissue-derived mesenchymal stromal cells - was superior to any other, or to a corticosteroid injection control, on pain at 12 months.

FDA states in its consumer alert that the only FDA-approved stem cell products in the United States are blood-forming stem cells derived from umbilical cord blood, approved for disorders of the blood-forming system, and that NONE of these products - stem cells, stromal vascular fraction, umbilical cord blood, Wharton's jelly or amniotic fluid - has been approved for the treatment of any orthopedic condition, including osteoarthritis, tendonitis, disc disease, tennis elbow, back pain, hip pain, knee pain, neck pain or shoulder pain.

California Stem Cell Treatment Center that stromal vascular fraction produced by enzymatically digesting a patient's own fat IS a drug under the Federal Food, Drug, and Cosmetic Act, and that the same surgical procedure exception does not apply because the removed fat and the implanted SVF are not the same thing. In 96 patients, concentrated bone marrow aspirate contained a mean of 26.3 million nucleated cells per mL but only about 1,422 colony-forming units of connective tissue progenitors, and the nucleated cell count had negligible predictive value for how many progenitor cells were actually present (r-squared 0.28).

## What should I notice about the soreness?

Write down the ache's location and the movement that restarts it. Also note whether rest, easy motion, or a shorter activity helps.

Joint wear can make weight harder to spread across a knee or hip. Nearby muscles or a tendon can also become sore after added work.

## What can I try at home first?

Reduce the activity that starts the soreness, but keep comfortable movement. Complete rest isn't usually needed and may leave the joint stiffer.

Increase strength work in small amounts and check the joint tomorrow morning. Skin medicine, a brace, or a cane may also help.

## When should I seek prompt care?

A hot, swollen joint with fever needs prompt care. After an injury, get help soon if you can't stand on the leg.

Loss of bladder or bowel control, weakness, or numbness is urgent. After a procedure, call about spreading redness, rising soreness, or fever.

## What should I expect at a clinic visit?

The provider asks when the soreness began and which movements bring it back. You may be asked to walk, bend, reach, or turn.

Take your current medicines and earlier joint reports. The exam helps compare more home care, an office procedure, or a surgery visit.

## What should I know about PRP and concentrated PRP?

PRP starts with your blood, which is spun to gather more platelets. The prepared blood portion is then put into your sore joint.

Platelets are tiny pieces in blood that help it clot. The concentrated form contains more platelets, but that doesn't prove greater help.

## What can QC Kinetix offer if soreness doesn't settle?

QC Kinetix calls its blood- or marrow-based office procedures regenerative treatments. PRP and concentrated PRP use blood, while a marrow procedure begins at the pelvis.

The clinic calls the person doing the exam and procedure a medical provider. Ask if you'll see a doctor, then request the exact steps, cost, aftercare, and follow-up.

## What to do when soreness stays

When soreness stays, QC Kinetix offers regenerative treatments, its name for office procedures using your blood or marrow without surgery. The medical provider does the exam and procedure; confirm whether you'll be treated by a doctor.

Take your current medicines, earlier joint reports, and notes about daily limits. The Chandler office is at 1100 S. Dobson Road, Suite 210; call (602) 837-PAIN.
